A porção diafisária será absorvida pelo organismo meses após sua implantação, tempo necessário para a fixação definitiva da parte metafisária em sua posição adequada através da osteointegração. Este conjunto permite o perfeito alinhamento da prótese durante o procedimento cirúrgico servindo a haste diafisária como guia dentro do canal femoral, e devido ao seu pequeno tamanho facilitará a retirada da parte superior caso uma cirurgia de revisão seja necessária.

"PRÓTESE FEMORAL COM HASTE ABSORVÍVEL"
O presente objeto refere-se a um componente femoral de uma prótese de quadril constituído por duas partes, uma porção superior (metafisária) fabricada em material biocompatível não bioabsorvível e uma porção inferior (diafisária) bioabsorvível. A porção diafisária será absorvida pelo organismo meses após sua implantação, tempo necessário para a fixação definitiva da parte metafisária em sua posição adequada através da osteointegração. Este conjunto permite o perfeito alinhamento da prótese durante o procedimento cirúrgico servindo a haste diafisária como guia dentro do canal femoral, e devido ao seu pequeno tamanho facilitará a retirada da parte superior, caso uma cirurgia de revisão seja necessária.
As próteses de quadril atuais tem por função substituir a articulação coxo- femoral nas condições em que o quadril está deteriorado por doenças ou por fraturas.
Atualmente existem próteses totais ou parciais, onde a porção acetabular da articulação pode ou não ser substituída. A parte femoral pode ser cimentada ao osso ou não, sendo que as próteses que não necessitam do cimento ósseo para fixar-se ao osso estão sendo progressivamente mais utilizadas. Embora tenham múltiplas vantagens, estas próteses não cimentadas acarretam problemas que se busca m contornar através de várias modificações.
Basicamente existem dois tipos de componentes femorais - os componentes padrões, longos, compostos por uma porção metafisária e uma porção diafisária (modulares ou inteiriças), e os componentes metafisários, curtos, apenas com a porção mais superior, metafisária.
Dentre todos os procedimentos médicos, a artroplastia total do quadril é considerada o de melhor resultado se medido pela satisfação do paciente, entretanto as próteses clássicas possuem uma haste no segmento femoral que é fixada no interior do canal deste osso através de cimento ou fixação biológica (o próprio osso fixa-se em microporosidades na superfície da prótese). Esta fixação rígida provoca por vezes os problemas citados a seguir:
1. Dor na coxa devido a diferença entre o módulo de elasticidade do material metálico e o osso;
2. Stress shielding, que é a reabsorção óssea da região proximal do fémur devido à falta de estímulo mecânico sobre essa região, causado pela transferência de carga para a prótese e desta para a parte distai do fémur, fragilizando o osso proximal não estimulado;
3. Na hipótese de ser necessário retirar uma prótese fixa em uma segunda cirurgia (revisão), este procedimento torna-se longo e difícil, com grande sangramento e por vezes com fratura do osso.
Para resolver esses problemas foi criado uma prótese chamada metafisária, que não tem a haste diafisária, sendo fixa apenas na região proximal (metafisária) do fémur.
Outros problemas surgiram pela falta da haste diafisária:
1. Posicionamento incorreto pelo cirurgião, pois a haste diafisária serve de guia, orientando o correto posicionamento da prótese;
2. Problemas com a estabilidade imediata da prótese (falta de press-fit), pois a área de fixação mecânica (não biológica) é muito pequena podendo levar a mudanças da posição da prótese e consequente falha da cirurgia;
3. Falta da fixação biológica óssea, pois como a prótese é muito pequena pode permitir micromovimentos, impedindo o crescimento e fixação do tecido ósseo à superfície da prótese, também levando à falha da cirurgia.
A presente invenção contorna os três problemas inicialmente citados nas próteses tradicionais e evita os três problemas citados na prótese metafisária, mantendo as vantagens dos dois implantes, pois consiste de uma prótese femoral com uma haste diafisária absorvível pelo organismo e como esta absorção ocorre após o período necessário para a fixação biológica, a parte diafisária desaparecerá mantendo a parte metafisária fixa em uma posição adequada.
As outras características da invenção são:
• Colo com formato cónico em sua extremidade proximal que possibilita o uso de cabeças femorais de diferentes tamanhos e materiais.
• Colar para apoio sobre a cortical medial do fémur aumentando a estabilidade rotacional e impedindo o afundamento da prótese.
• Ângulo no perfil lateral entre as porções metafisária e diafisária adequado à anatomia do canal femoral
• Furo roscado para fixação entre a porção diafisária e metafisária através de recursos de fixação.
• Cavidade na porção metafisária que permite o encaixe da parte absorvível aumentando a rigidez do sistema.
• Vários tamanhos para adaptar-se às particularidades anatómicas de todos os biótipos.
• Perfis lateral e frontal cuneiformes para encaixe estável através do uso de instrumental cirúrgico que compacta o interior do fémur dando ao canal a mesma forma da prótese.

A Figura 4 é um corte parcial da montagem mostrando os furos roscados internos das duas peças.
A Figura 5 apresenta a vista frontal da porção metafisária e a vista lateral em corte da mesma.
A Figura 6 mostra a vista frontal da porção diafisária e a vista lateral em corte da mesma.
De acordo com a Figura 1 e 2, consiste a presente invenção de uma prótese femoral (1) preferencialmente em metal de superfície porosa e altamente biocompatível, mais preferivelmente revestido com uma camada fina de hidróxi- hapatita (osso artificial), sendo que a sua porção diafisária (2) é composta por um material biocompatível de absorção lenta (como polímeros naturais ou sintéticos, materiais inorgânicos ou combinações destes). Como na Figura 3, estes dois componentes (1) e (2) são fixos entre si e possuem um colo cónico (3) na região proximal (1) que permite o encaixe de cabeças femorais de diferentes tamanhos.
Como não é utilizado cimento para sua fixação, a prótese terá diferentes tamanhos e formatos para adaptar-se a anatomia dos mais diversos pacientes e um colar (4) (Figura 3) para o apoio sobre a cortical medial do fémur aumentando a estabilidade rotacional e impedindo o afundamento da prótese. Seu formato será preferencialmente cuneiforme para que o encaixe da prótese no canal femoral fique estável. De acordo com a Figura 4, é possível observar o ângulo (5), também adequado a anatomia do canal femoral.
A fixação entre os dois segmentos (1) e (2) será por meio de recursos de fixação que serão instalados nos furos (6) e (7) (Figura 5 e 6) ou por materiais adesivos adequados para os esforços solicitantes ou por meio de combinações dos dois métodos. Na eventualidade de ser necessária a remoção da porção metafisária (1), este furo possuirá um perfil roscado (8) na sua parte superior que servirá para encaixe de um extrator com a ponta também roscada. Para todas estas finalidades os furos (6), (7), (8) terão diamêtros diferentes ao logo de seu comprimento, com partes roscadas ou não.
De acordo com a Figura 6, na porção diafisária (2) haverá uma protuberância (9) na parte superior com um furo roscado (7) que servirá para o encaixe e a fixação das duas partes. Para o encaixe será necessário um canal (10) com a mesma geometria da protuberância na porção metáfisaria (1).
Claims
1. Prótese femoral com haste absorvível caracterizada por uma porção superior (1) (metafisária) fabricada em material biocompatível e uma porção inferior (2) (diafisária) absorvível, fixadas entre si preferencialmente por recursos de fixação e/ou materiais adesivos.
2. Prótese femoral com haste absorvível de acordo com a reivindicação 1, caracterizada por ter diferentes medidas e formatos a fim de atender diferentes populações e se adaptar ao canal femoral.
3. Prótese femoral com haste absorvível de acordo com a reivindicação 1 e 2, caracterizada por furos passantes (8) e (9) ao longo da porção metafisária (1), preferencialmente com variações em seus diâmetros, e menos preferencialmente sem variações em seus diâmetros para a colocação de recursos de fixação e um extrator ou posicionador quando necessário.
4. Prótese femoral com haste absorvível de acordo com a reivindicação 1 a 3, caracterizada por ter um formato (9) na parte superior da porção diafisária que encaixe (2) no canal (10) da porção metafisária (1) para a fixação de ambas as partes.
